All you have to do is sew a pouch of sorts and fill with rice and something like lavendar would be great to relax her. Tuck in the micro for a few and then lay on her ear.
As far as the medicine, sometimes you just have to force it. I know its heartbreaking.

When my oldest son was young, he had horrible ear infections. About taking the medications, even when he was 4 or 5 years old, we would use a syringe and put it far back in his cheek so he couldnt spit it out.
We personally used ear candles and they seemed to help cut down the frequency of Jared's ear infections. I hope Bella feels better soon.

One of my friends made a bunch of us rice bags for Christmas one year. She used hand towels folded over, and they're great. So, besides the sock, that's something else you could use or a wash cloth, just to make a small one. Hope she feels better soon

I have a spitter as well. After Rose had her tonsils and adnoids taken out she refused any meds. We tried everything. I hope you can find something with Bella that works.

You can use an inexpensive washcloth. Fold it in half, stitch up the sides leaving a snall opening. Fill with about a 1/4 cup of regular rice, not the Uncle Ben's! Finish stitching up the opening and you'll have a nice sized for small children's ears that has a double layer to prevent the rice grains from excaping. Nuke for a few minutes to warm, but check before allowing the baby to use it.

My grandmother made them and used a birdseed mixture that has corn in it and it smells of popcorn when heated. But she sewed 2 washcloths together and made larger one.

At our pharmacy, they will add flavorings to the medicine for small children, they have added bubble gum, grape and strawberry flavorings to any liquid meds our kids take.
